Understanding Long Covid

Long Covid has become the major public health threat of our era, impacting at least 400 million people globally. Each time a person develops Covid-19, the risk of long-term complications increases — regardless of vaccination status. Remarkably, 90% of people with persisting symptoms had a mild initial illness.

Unlike most post-viral syndromes, Long Covid is distinctly different because it becomes more than just the persistence of initial symptoms. Covid enters as a respiratory infection but essentially becomes a circulatory disease — the virus has a high affinity for cells lining blood vessels.

The Two Core Issues

ACE2 Deficiency

Covid virus attaches to ACE2 — a vital enzyme — and damages it. Virtually all complications of Covid-19 can be traced to ACE2 deficiency. ACE2 damage causes cardiovascular problems, blood clots, loss of blood vessels, organ damage, and increased inflammation.

Mitochondrial Dysfunction

As the virus destroys ACE2, it also damages mitochondria — the powerhouses generating 90% of cellular energy. Even after a mild infection, mitochondrial dysfunction can continue for months, causing fatigue, brain fog, and immune impairment.

Good news: Restoring ACE2 and rescuing mitochondrial function are the foundation for healing from Long Covid — and there are many evidence-based ways to do this.

The Web of Long Covid

Dr. Galland developed this visual metaphor to explain the interconnected dysfunctions that drive Long Covid. Every strand connects to every other — addressing one part often helps the whole.

1. Endothelitis

Inflammation of blood vessel lining, causing capillary loss and vascular stiffness. Restricts blood flow and oxygen delivery.

2. Microthrombosis

Tiny blood clots clogging capillaries. Different mechanism from ordinary clotting — standard anti-clotting measures often don't work.

3. Mast Cell Activation (MCAS)

Primitive immune cells releasing inflammatory chemicals. Causes unpredictable reactions to many treatments and foods. Often the pivotal factor in treatment-resistant Long Covid.

4. Monocyte Polarization

Disrupted immune cell lifecycle creating chronic, unrelenting inflammation. ACE2 and mitochondrial support are primary corrective interventions.

5. Auto-Antibodies

Antibodies attacking the body's own cells. Many Covid-induced autoantibodies become active only when inflammation and tissue damage are present — controlling inflammation is the best defense.

6. T-Cell Impairment

SARS-CoV-2 directly invades and disables T-lymphocytes — the generals of the immune system. This also allows dormant viruses like Epstein-Barr to reactivate.

7. Viral Persistence

Persistence of SARS-CoV-2 or its proteins (demonstrated up to 2 years post-infection) drives ongoing inflammation. Main reservoir appears to be the gastrointestinal tract.

8. Gut Microbial Dysbiosis

Long Covid patients had already lost beneficial gut bacteria before developing Long Covid. Restoring a healthy microbiome is crucial for full recovery and immune health.
